Hello Roman, thanks a lot for helping me and sorry for not being specific enough concerning the contact form.
I installed the plugin and entered the code, but it looks quite different. what did I do wrong?
btw, as you can see I switched to sydney theme, but I guess this has nothing to do with it, has it?

Well, I think that there is no easy way to make it look the same in Sydney theme, unfortunately. Sydney theme has its own CSS styles.
